For all adults, your first visit with us will include a detailed New Patient Comprehensive Exam and Full Set of X-Rays Appointment before seeing a hygienist.
Once you arrive to your appointment, you are welcome to relax in our patient waiting area. Mask requirements may vary depending on local community COVID-19 levels. If you would prefer to wait in your vehicle, please call or text the office letting us know what vehicle you are in, and your provider will escort you directly to your appointment chair.

Our online paperwork is preferred and encouraged. Using our online portal is the easiest and fastest way to complete your forms. If you would rather complete printed paperwork, printable patient forms may be downloaded below. Completed forms can be brought in during your visit or emailed to springcreekdds@gmail.com.
